CHILDHOOD IMMUNIZATION CLINICS CALL: 574-523-2127
All Appointments are Scheduled through the Elkhart Office.IMPORTANT:
- Child must be accompanied by parent/guardian or an adult 18 years or older with written permission from parent.
- Complete shot record showing all immunizations received since birth should accompany child.
- $8.00 per vaccination. No one refused for inability to pay.
Vaccines are given based on guidelines from the Indiana State Department of Health (ISDH) and the Centers for Disease Control and Prevention (CDC).

IMPORTANTE:
Traiga con Ud. Lo sigiente a cada cita:
Su historia de vacunas.
Permiso Firmado del padre/guardian si otra persona (tiene que ser sobre 18 anos) trae el nino a la clinica y no hay una previa forma firmada.
$8.00 dolares cada vacuna, no refusan nadien si no pueden pagar.
Apreciamos si puedas traer un inteprete cuando vengas a la cita.
